include assisting clients with setting up household, tasks related to ensuring home is tidy and has enough food. Clients will be able to do shopping (as appropriate); meal preparation; managing bills (with or without help) and communicate needs around these life areas.
Utilizing resources within the community at large as well as the client's identified core supports, along with ARMHS philosophy, client needs will be assessed and resources will be engaged as needed to reduce likelihood of additional hospitalization, detox or a more restrictive living environment.
In an attempt to foster autonomous and prevent relapse, (mental, emotional, physical) clients, clinical team and client support members will work towards the goal of transitioning a client into more autonomous living while reducing the risk of destabilization. Adequate time will be given when planning such transition so that everyone involved can provide input and support the client in a transition timeline which meets the client's needs and also prepares the client for a successful new living environment.
These services will be delivered by CPS individuals who will set up regular (sometimes daily) contact with the client to help client stay focused on routine recovery tasks and assist the client with skill development as needed and helping to hold client accountable for task deadlines.
We are committed to the always customized delivery of care to meet the client's current needs to stabilize and maintain his/her life situation. Therefore, regular functional assessments (4), individual treatment planning (5), and medication education (6) will be a routine part of Stride's delivery model. Multidisciplinary team input is used as well as referrals and partnerships with outside professionals can and will be included in the delivery of care.
